LOG SHEETS 

Students must maintain a log sheet throughout the summer. Students should duplicate this form as needed. Students are responsible for recording their daily hours and activities at their placement site. The log is to be an accurate reflection of the days and hours spent at the internship site. It is the student’s responsibility to ensure that the log is signed by the mentor and submitted online to the MAST Academy Summer Internship Instructor by the deadlines indicated in the Program Calendar.

How to submit your log sheet online:

1) Download the form listed above. 

2) Rename the file using this format: 2012-LastName-FirstName-LogSheet

3) You may fill out the log sheet by hand or by computer.

4) Have your mentor sign your log sheet.

You may use the Echosign Service (www.echosign.com) to submit your log sheet to your mentor. S/he will sign off on the log sheet digitally and send it back to you.  Once it has been signed digitally, you may upload it via DROPBOX. OR

Make a clear scan of the log sheet with your mentor's signature and upload it via DROPBOX. Be sure to name the scanned file using the "2012-LastName-FirstName-Logsheet.jpg" or pdf format.

OR

Make a clear READABLE digital photograph of the log sheet with your mentor's signature and upload it via DROPBOX. Be sure to rename the photograph file using the "2012-LastName-FirstName-Logsheet.jpg" format.

5) Be sure to submit log sheet by the deadline noted in the calendar.
